REGISTRATION AND DROP/ADD PROCEDURES

Procedures regarding initial registration and subsequent changes in schedules for students in Columbia and Greenville are outlined below. All forms are available on-line or in the School of Medicine Office of Admissions and Enrollment Services.  The deadline to drop or add is four (4) weeks prior to the beginning date of the elective.  All forms must be on file by this time.  Please note that GHS students submit all forms, applications, etc., directly to personnel in the Office of Medical Student Services at GHS who then forward them to the USCSM Director of Enrollment Services/Registrar in Columbia. 

1. USCSM M-IV Electives

To Register:  List the desired elective on the Schedule Request Form.

To Drop:  Have the Request to Drop/Add M-IV Elective Form
signed by the course director of the elective you wish to drop and return it to the Director of Enrollment Services/Registrar.

To Add:  Contact the Director of Enrollment Services/Registrar regarding availability of space. Have the Request to Drop/Add
M-IV  Elective Form
signed by the course director of the elective you wish to add. Return it to the Director of Enrollment Services/Registrar.

2. SCAHEC Elective

To Register:  Complete a South Carolina AHEC Student Information Form and return it with your Schedule Request Form.

To Drop:  Notify the Director of Enrollment Services/Registrar.

To Add:  Contact the Director of Enrollment Services/Registrar and follow procedures to register.

3. Independent Study Electives

To Register:  Complete a Request to Take M-IV Independent Study Elective Form and obtain the approval and signature of the faculty sponsor, the department chair, and the Assistant Dean for Clinical Curriculum.  Return it with the Schedule Request Form.

To Drop:  Notify the faculty sponsor, the department chair, and the Director of Enrollment Services/Registrar.

To Add: Follow procedures to register.

4.. Extramural Electives in the United States and Canada

To Register:  Contact the host institution for information and an application. Bring the completed application to the USCSM Director of Enrollment Services/Registrar to have the school section filled out. Complete a Request To Take Extramural Elective; have it approved and signed by the respective department chair and the Assistant Dean for Clinical Curriculum; return the form to the USCSM Director of Enrollment Services/Registrar. When you have confirmation from the host institution, documentation must be provided to the USCSM Director of Enrollment Services/Registrar. A student is not registered for the elective until confirmation is received and the request form has been completed.

If the elective is not at an LCME-accredited institution, the student must obtain and submit for approval to the respective department chair and the Assistant Dean for Clinical Curriculum the following items: letter of acceptance and dates from sponsoring school, elective description, exact description of student's responsibilities, and the name, address and telephone number of the elective coordinator.

To Drop:  Notify the institution and the USCSM Director of Enrollment Services/Registrar and provide the USCSM Director of Enrollment Services/Registrar with documentation of notification of cancellation to the extramural institution.

5. International Extramural Electives

To Register:  Contact the host institution for information and an application.   Bring the completed application to the USCSM Director of Enrollment Services/Registrar to have the school section filled out.  Complete a Request To Take Extramural Elective Form; have it approved and signed by the respective USCSM department chair and the Assistant Dean for Clinical Curriculum.   When you receive confirmation from the host institution, documenation must be provided to the USCSM Director of Enrollment Services/Registrar. A student is not registered for the elective until confirmation is received and the request form has been completed.
